IUNI Forms
IUNI staff welcomes input from the community of IU researchers we serve; in fact, communication across the IU research community is one of IUNI’s core functions. To facilitate communication with IUNI staff, we have created forms for the most common types of requests we receive. Filling out the forms allows us to track and manage all requests, ensuring they are attended to in a timely manner. All forms are intended for use by IU employees and require CAS login. For requests emanating from outside IU and for all inquiries and requests not covered in one of the forms listed, please use the Contact Us form.
